Registration Requirements
Foreign Students, Guardianships, Immunizations
Registration:
    To enter kindergarten, children must be 5 years old on or before Sept.1.  To enter first grade, children must be 6 years old on or before Sept.1.
To register your child in school, the following documentation is needed:
1.  Birth certificate
2. Proof of immunizations on a Form 680, which can be obtained at the Department of Health at the following address:

3.  Proof of physical examination within that last year.  If documentation cannot be provided, a physical examination must be scheduled within 30 days.
4.  Verification of your residency in Orange County with one of the following:
A.  Homestead Exemption Card
B.  Lease/Rental Agreement
C.  Notarized Statement
D.  Declaration of Domicile (for new residents of Florida)
To obtain a Declaration of Domicile ($8), apply at the Comptroller' Office, Monday through Friday, 7:30 a.m.-4:30 p.m., at the address below:

FOREIGN STUDENTS
            Foreign students who wish to change their status from non-student to student should contact Virginia Redding, Foreign Student advisor, Pupil Assignment, at 407-317-3200, Ext, 2114.
                       Please note: School records of foreign students must be translated into English. GUARDIANSHIPS
    If you are not the parent or custodial parent of a student, state law requires that one of the following documents be provided for enrollment:
            1.  Court Custody Documentation
            2.  Department of Children and Families Palcement Letter
            3.  Educational Guardianship (given only to parents or custodial parents living ouside Orange County)
